The provision of eye care in the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in the framework of the Program of state guarantees of free medical assistance to citizens of the Republic of Bashkortostan is based on the standards of medical care in accordance with the terms and conditions of the provision of eye care approved in the established order. 

1.1. The conditions of implementation established by the legislation of the Russian Federation of the right to choice of doctor (subject to his consent)

To obtain in the framework of specialized eye care in a planned form in SBI "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" choice of the medical organization is in the direction of the attending physician. In this case the attending physician must inform the citizen about the possibility of a medical organization taking into account the implementation conditions of the provision of eye care established by the Program.

Eye care in urgent or emergency form is provided to citizens with regard to compliance to the time frame for delivery.

When selecting the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" and the doctor has the right to receive information in an accessible form, including posting in the information-telecommunication network Internet, SBD "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus", to carry out its medical activities, as well as physicians about the levels of their education and qualifications.

The choice of the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" doctor of servicemen and persons equated medical support to military personnel, citizens performing alternative civil service by citizens subject to conscription for military service or sent for alternative civil service, and citizens entering military service under the contract or any equivalent service, as well as detainees in detention serving the penalty of restriction of liberty, arrest, imprisonment or administrative arrest, is subject to articles 25 and 26 of the Federal law "About bases of health protection of citizens in Russian Federation".

In the provision of citizen eye care Programme a range of medical organizations (except the provision of eye care in emergencies) outside of the Republic of Bashkortostan is carried out in the manner prescribed by the Ministry of health of the Russian Federation. 

1.2. The order of realization established by the legislation of the Russian Federation of the right to an extraordinary provision of eye care to separate categories of citizens

Eye care to separate categories of citizens is provided in SBI "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in accordance with the legislation of the Russian Federation out of turn.

The basis for the provision of eye care in the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" out of turn is a document confirming belonging of the citizen to one of the categories of citizens who, in accordance with the legislation of the Russian Federation granted an extraordinary medical assistance.

The right to extraordinary provision of eye care have the following categories of citizens:

1) Heroes of Socialist Labour; Heroes of labour of the Russian Federation, full gentlemen of order of Glory; heroes of the Soviet Union; heroes of the Russian Federation; members of families of Heroes of the Soviet Union, heroes of the Russian Federation and full gentlemen of order of Glory; full gentlemen of order of Labour Glory; widows (widowers) of Heroes of Socialist Labour, Heroes of Labour of the Russian Federation or full gentlemen of order of Labour Glory, not remarried (regardless of the date of death (death) of the Hero of Socialist Labour, Hero of labour of the Russian Federation or the full cavalier of the order of Labour Glory);
2) disabled veterans; 
3) participants of the great Patriotic war;
4) combat veterans;
5) servicemen, passing military service in military units, establishments, military educational institutions, were not part of the army, in the period from 22 June 1941 to 3 September 1945 for at least six months; military personnel awarded by awards or medals of the USSR for service during the specified period;
6) persons awarded by a sign "Inhabitant of blockade Leningrad";
7) former underage prisoners of concentration camps, ghettos and other places of forced contents created by fascists and their allies during the Second world war;
8) persons who worked during the great Patriotic war on objects of antiaircraft defence, local antiaircraft defence, building of defensive works, military-sea bases, airdromes and other military facilities within rear borders of operating fronts, operational zones of operating fleet, on front-line sections of Railways and roads;
9) members of the families of victims (died) disabled veterans, participants of the great Patriotic war and veterans of combat operations;
10) the persons awarded by a breastplate "the Honourable donor of Russia";
11) citizens exposed to radiation due to radiation accidents;
12) citizens recognized as victims of political repressions;
13) rehabilitated persons;
14) orphans and children left without parental care;
15) invalids I and II groups and disabled children.

Information about the categories of citizens having the right to extraordinary provision of eye care, located in the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in the stands and on the website of the institution.

At the address of the citizens having the right to extraordinary provision of eye care, the receptionist SBD "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" the record of the patient to the doctor out of turn. If necessary, perform diagnostic examinations and therapeutic procedures the attending physician will arrange for their provision as a priority. Providing routine inpatient eye care and outpatient eye care in day hospitals to the citizens having the right to extraordinary provision of medical care is performed outside the main sequence. The decision on the extraordinary provision of eye care clinic accepts the head of the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" on recommendation of the doctor, providing primary and specialized health care, making an appropriate entry in the list. 

1.3. The procedure for providing citizens with medicines, medical products, included in approved by the Government of the Russian Federation the list of medical devices implanted in the human body, the donor of blood and blood components, clinical nutrition, including specialized products of medical nutrition, for medical reasons in accordance with the standards of eye care with the exception of clinical nutrition at the request of the patient

When rendering through the Provision of specialized eye care, including high-tech, health care, eye care in urgent or emergency form, is to provide citizens with drugs for medical use, included in the Territorial list of vital and essential medicinal drugs used in providing hospital care, medical care in day hospitals and emergency medical care under the Program (in accordance with Appendix # 6 to the Program of state guarantees of free medical assistance to citizens of the Republic of Bashkortostan for the year 2018 and the planning period of 2019 and 2020) and medical devices, provided that standards of medical care.

Provide citizens with blood and its components is carried out in the provision of specialized ophthalmological care in a hospital when there are medical indications in compliance with standards of care.

Providing citizens with medical products included in the list of medical devices implanted in the human body when providing medical care within program of state guarantees of free rendering to citizens of medical aid, approved by the decree of the Government of the Russian Federation from October 22, 2016 No. 2229-p (as amended by the decree of the Government of the Russian 13 Federation from July 25, 2017 No. 1587-R), at the expense of means of the Federal budget, the budget of the Republic of Bashkortostan and at the expense of compulsory medical insurance in accordance with the law.

Direct implantation of medical devices is carried out after obtaining voluntary consent from the patient or his legal representative for appropriate medical intervention. 

1.4. The list of measures to prevent diseases and promote healthy lifestyles, carried out in the framework of the Program

In the system of measures for the prevention of diseases and promotion of healthy lifestyles includes:

the publication of information and methodological materials aimed at the propaganda of healthy lifestyle among the population;
banners, billboards, posters and other visual propaganda (full color), the creation and broadcast of television video clips and films dedicated to a healthy lifestyle;
equipment and supplies centres and branches (offices) medical prevention health organizations;
organization and conducting of lectures, talks in schools and health weeks health conferences, round tables, competitions for youth, is dedicated to the promotion of healthy lifestyles, prevention of drug addiction, alcoholism, Smoking;
conduct national events to promote healthy lifestyles; conducting surveys of citizens to identify risk factors of diseases development. 

1.5. Waiting periods medical assistance in routine and emergency form 

The GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in the provision of eye care in the outpatient setting:
provided by the coincidence of the time of reception of the doctor-the ophthalmologist hours of offices and services, carried out consultations, research, procedures;
the time allotted for the reception of the patient, is determined in accordance with normative legal acts approved in the prescribed manner;
the waiting time of rendering primary specialized medical and sanitary care in the emergency form should not exceed 2 hours of treatment;
the waiting time of medical specialists in rendering primary specialized eye health care in a planned form should not exceed 14 calendar days from the date of treatment;
the waiting time of the diagnostic tool (functional diagnostics, ultrasound) and laboratory research in the provision of primary health care in planned form should not exceed 14 calendar days from the date of appointment;

The GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in the provision of medical care in stationary conditions:

medical assistance around the clock;
the waiting time in the provision of specialized, with the exception of hi-tech medical care in a hospital in the routine should not exceed 30 calendar days from the date of issuance by the physician of referral for admission (subject to the patient's application for admission to a recommended physician time), and for patients with cancer - not to exceed 14 calendar days from the date of histological verification of the tumor or the diagnosis of the disease (condition).

The waiting time of high-tech medical care in a hospital in the planned form is installed in accordance with the legislation of the Russian Federation.

The GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in the provision of specialized medical care in stationary conditions, is the waiting list provide specialized eye care in a planned form and is to inform citizens in an accessible form, including with use is information-telecommunication network Internet, the waiting time in the provision of specialized ophthalmologic care tailored to the requirements of the legislation of the Russian Federation on personal data. 

1.6. The conditions of stay of citizens on free medical aid in stationary conditions

Hospitalization of the patient is carried out in accordance with the medical testimony in the direction of a physician medical organizations regardless of ownership and departmental affiliation, medical emergencies, and also when self-referral of the patient for emergency indications.

Patients are provided with therapeutic food in accordance with the norms approved by the order of Ministry of health of the Russian Federation of 21 June 2013 No. 395н and within the stipulated financial resources.

For child care, regardless of age, one parent, other family member or other legal representative entitled to free being together with the child in the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" at rendering to it medical aid in stationary conditions during the entire period of treatment. When the joint presence in the GBU "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus" in stationary conditions with a child under the age of four years (with a child over this age – at presence of medical indications) charge for creation of conditions of stay in stationary conditions, including the provision of bed and food, these individuals will not be charged. 

1.7. The conditions of accommodation of patients in small wards (boxes) for medical and (or) of epidemiological indications

The GBU "Ufa research Institute of eye diseases of the Academy of Sciences RB" conditions for the possibility of the patient's visit and stay with him in a medical organization of relatives taking into account the patient's condition, compliance with anti-epidemic regime and the interests of other persons, working and (or) Institute.

For medical and (or) of epidemiological indications established by the Ministry of health of the Russian Federation, placing patients in small houses (boxes) shall be in accordance with the order of the Ministry of health and social development of the Russian Federation of 15 may 2012 No. 535н "On approval of list of medical and epidemiological indications for placing patients in small houses (boxes)". Mandatory conditions of stay in the small capacity chamber (the box) are isolation of patients from external factors, and in cases of infectious diseases – prevention of infection of others, observance of the sanitary-hygienic norms and regulations in the cleaning and disinfection of premises and surrounding objects in small chambers (boxes).

1.8. The procedure for the provision of transportation services when accompanied by a medical worker a patient undergoing treatment in stationary conditions

In order to fulfill orders for the provision of medical care and standards of care to the patient undergoing medical treatment in a hospital, if necessary, conducting diagnostic studies in the absence of their feasibility, SBD "Ufa research Institute of eye diseases of the Academy of Sciences of Belarus", providing medical services to the patient, and is transportation Institute, accompanied by the medical worker. 

1.9. The conditions for granting to orphan children and children left without parental care, in case of detection of disease medical care of all kinds, including specialized, including high-tech, medical aid

In case of revealing of children-orphans and children left without parental support (further – the minor), diseases that require medical assistance of all types, including specialized, including high-tech, medical aid, such minor medical care of all kinds, including specialized, including high-tech, medical aid, medical rehabilitation, Spa treatment and clinical supervision are provided on a priority basis.
